If you haven’t yet decided whether to rebuild, or who to rebuild with, or who might be handling the cleanup, this form is crucial.

Why is it important?

  • If you have insurance, the Army Corps of Engineers will invoice your insurance for their services, and they will receive payment based on your insurance coverage.
  • If you don’t have insurance, these services will be provided to you free of charge—but only if you sign up by March 31st!
